Terms and Conditions of Mascot Matchmakers Event
Including Release and Waiver of Liability

1. Purpose of Event and Disclosure. The Iowa State University Alumni Association (“ISUAA”) and the alumni associations of the University of Northern Iowa (“Northern Iowa Alumni Association”) and University of Iowa (“Iowa Alumni Association”) are providing single alumni and/or friends of Iowa State University, University of Iowa and University of Northern Iowa an opportunity to meet other single people in the Des Moines area. This event is known as the “ISU-UNI-Iowa Speed Dating” (hereinafter “Mascot Matchmakers”). Mascot Matchmakers is not a matchmaking service. Neither ISUAA, Northern Iowa Alumni Association, nor Iowa Alumni Association perform a background check on any of the participants at Mascot Matchmakers.

2. Registration. By registering for Mascot Matchmakers you agree to the following terms and conditions (if you do not agree, please do not register for this event):
a.   You are solely responsible for the information you share with fellow participants at the Mascot Matchmakers as well as your conduct toward participants. ISUAA, Northern Iowa Alumni Association, and Iowa Alumni Association reserve the right to resolve any situations causing participants discomfort.
b.       You agree to allow the ISUAA, UNI Alumni Association and/or Iowa Alumni Association to share your contact information with your matches for purposes of potentially being contacted as a result of participating in this event.
c.      You agree not to share personal contact information (including e-mails, phone numbers, business cards, etc.) during the event. All matches will be confirmed through the respective alumni associations who will, in turn, contact all similar matches after the event.
d.    You understand that neither ISUAA, UNI Alumni Association nor Iowa Alumni Association makes any representation or warranty as to the conduct of other participants.

3. Release and Waiver of Liability.

PLEASE READ THIS CAREFULLY.
It affects any rights you may have if you are injured or otherwise suffer damages while participating in Mascot Matchmakers.

I, a participant in Mascot Matchmakers hereby release, waive, discharge and covenant not to sue ISUAA, Iowa State University, University of Northern Iowa, University of Northern Iowa Alumni Association, University of Iowa, University of Iowa Alumni Association, the State of Iowa, the Board of Regents of the State and any of the officers, servants, agents, volunteers and employees of any of the above-mentioned entities (hereinafter referred to as RELEASEES) for any liability, claim and/or cause of action arising out of or related to any loss, damage or injury, including death, that occurs as a result of my participation in the above-described activities.

I agree to indemnify and hold harmless the RELEASEES whether injury is caused by my negligence, the negligence of the RELEASEES or the negligence of any third party. I further agree that this Release and Waiver of Liability shall bind the members of my family and spouse, if I am alive, and my heirs, assigns and personal representatives, if I am deceased, and shall be deemed as a RELEASE, WAIVER, DISCHARGE AND COVENANT NOT TO SUE the above-named RELEASEES. I hereby further agree that this Release and Waiver of Liability shall be construed in accordance with the laws of the State of Iowa.

By agreeing to this Terms and Conditions of Mascot Matchmakers Including Release and Waiver of Liability, I state that I have read and understand the conditions set forth herein, including those contained in this Release and that I agree to all conditions set forth herein, and that I agree to this voluntarily.
